About this project

Frailty Feedback is designed around a short questionnaire that can be opened on a smartphone by scanning a QR code. Staff-facing review and governance tools are intended to be protected separately.

Planned experience

A responsive public questionnaire intended primarily for smartphones.
A protected dashboard for authorised staff feedback review.
A separate governance area for appropriate oversight and accountability.

Release and availability

The planned service address is feedback.xyrod.com. The service, public questionnaire and QR code are not being linked from this website until the required deployment, access-control, privacy and organisational governance checks have passed.
